Due to an oversight, the incorrect ZIP code was used on the downloadable Katie Award form. The error has been corrected, and the current form is correct. The NEW accurate ZIP code is 75219. You may download the new form or use the old form — just be sure to mail the submissions to the correct ZIP code. If any submissions have been mailed and are returned, please resubmit using the correct ZIP code. The board apologizes for the inconvenience.
